"Pure O" differs from regular OCD in that it does not have a clearly expressed physical component. Such OCD is more about obsessions (intrusive thoughts) than about compulsions (repetitive actions and rituals). A person with "pure O" does not use rituals to avoid intrusive thoughts, but unwanted images and ideas in the mind remain. And fighting them can be just as difficult as with regular OCD.
